Forms Available from the Grand Lodge:

The following group of forms will be furnished by the Grand Secretary's Office upon receipt of requests for same:

Forms: 38, 45, 50, 55, 56 and 60.

The following group are also furnished by the Grand Secretary's Office, but they are not sent to the lodges until the need arises or until petitions, requests, etc., are received which require their use:

Forms: 1, 2, 3, 4, 5, 6, 7, 8, 10, 11, 12, 13, 14, 15, 16, 24, 43, 46, 53, 58, 59, 67 and 68.
Please Note: Form 46 is a Certificate of Life Membership. We furnish this certificate upon proper notice of election. It is also furnished by the Printing Department. However, the certificate requires the certification of the Grand Secretary before presentation.
Form GS 35-A is a Certificate of Masonic Status and is furnished only by the Grand Secretary's Office. It is the equivalent of a Certificate of Good Standing and is used only for transfer of membership to a lodge in another state.
